The american dream is a lie, america is as uncivilized as the rest of the world, they're just better at hiding it. Nico has seen some real shit and it's left him a callous, broken man inside. He either develops/retains a soft spot of humanity or has given up on life and is now remorseless. The one small hope spot he eventually earns is violently crushed. By his own hands, even. It's actually a bit of a twist from standard videogame/GTA format, as the events of the game do little to help and "winning" simply means a prolonged empty, hollow joyless existence that makes death seem merciful.

It's the story of a man trying to find a place in a world that has no place for him, and his pointless struggle against his own nature. He's a monster who's seeking redemption, but the fact that america is as horrible as anywhere else just causes him to fall to old habits. Perhaps even further than before.

All buried under a veneer of fart jokes, toilet humor, and jokes about genitalia.

But in the end Nico finally confronts his past and finds resolution, instead of continuing to run from it. At the very least he managed to lay a foundation for a new life, despite the great amount of loss that it required. Still a pretty dark ending though.
