Actually, I did. I gained loyalty for all members, and resolved the disputes between Tali/Legion and Jack/Miranda.

Still lost Jack, Thane ,Legion, Kasumi and Zaheed. After 8 playthroughs, I've decided its random and loyalty has no effect. If you get through the mission with no loss, you got lucky.

NO. It's not random. You're putting people in the wrong places or not getting the ship upgrades. Garrus, Tali, and Jacob have mandatory upgrades.

Tubes = Tali/Legion/Kasumi
Second Team = Garrus/Zaeed ALWAYS.
Biotic = Samara/Jack
Escort = Mordin
