It most definitely does.

I'll probably do that one on Very Easy, and try and do it at the very beginning of the game or something. As far as I'm aware there isn't a time limit between each kill (like, it doesn't have to be a group of ten guys or anything), just that 10 kills in a row have to be Steel Fists. I could be wrong, though. I often am.

You are right, you can do it whenever you want via chapter selection in any difficulty level and it's not time based. The same for all the kill x enemies using this or that weapon. Also, all kills are cumulative.
The more you play the easier will be, and since you have to play on hard to unlock crushing, by that time you won't find crushing too crushing.
Now, if we were talking about Uncharted... poor Steve

Romero Would Be Proud... And it seems there was only about 4 million (if you run from the ones in the dark area in stage... 3? 4? The one that only has a couple of flares on the ground) in all 10 stages. Of course, considering that was enough to fully upgrade everything, buy ammo for everything, replenish said ammo a few times, and still have 800,000+ at the end on the character I was using....
