I lost Jack, Legion, Zaheed, Kasumi and Grunt, as well as all of the crew since I went ahead and did loyalties before the suicide mission. I had 100% loyalty, so I still maintain the deaths of squadmates is fully random.

Tali is exiled from the Migrant Fleet.

Yes. I did have some stop on me for some reason, but I just went to the list and clicked another, and started up again.

I believe that the only character who can die outside of no loyalty, and specialty selection is Mordin, ALWAYS SEND HIM BACK TO THE SHIP.
