I would have to say Dragonheart for the PSX...I liked the movie so I rented the game. I returned it an hour later.
Also the first VR Football for the PSX. I was weaned on Madden and I still have nightmares about that night that I rented VR. Oh the horror...

While not a video game, there is one hideous PC game that sticks out in my mind: Gadget. It was the most worthless game I've ever seen. Had the controls not been so difficult to figure out I could have easily finished the game in 15 minutes. No story, no puzzles, hardly any dialogue, no music, no point to playing it whatsoever. The only content it even had were a bunch of somewhat nicely rendered (still-frame) graphics. That game still gives me nightmares....

Although I like Simpons...they have yet to release a decent game. Take Simpsons: Road rage for example.......crap!
I would have to say that the worse game I played was the E.T game for the atari.
