That's a scene release, where the "original" files released by the scene are preserved by tossing them into another container (ie, a single .rar file). Now you're stuck because you can't tell which file from those files you want to extract from, because Windows is hiding known file extensions. Fix that, here's a random guide:

Once that's done ignore all the .r## files, spot the .rar file and extract from it. Winrar will pull the complete .iso file out of the spanned files.
