the patch puts a very small DVD code (DVD-Video as it happens) on the disc aswell as the game and the mod looks for this dvd code and boots the disc, but it's not official code otherwise the mod would not be needed, just the dvd patch
if you setup esr correctly you don't need to do anything it will auto boot up as soon as you pop in an esr patched PS2 DVD game, you can set it up so you need to manually tell the PS2 to boot it up.... but don't see the point in that
if you wanted to use codebreaker cheats you can also have a patched codebreaker.elf on a usb stick and have that boot up instead of the game first (when the usb stick is inserted) after you pick your codes and say start game the game will boot via ESR with cheats aswell
again the above only works that way if it's setup correctly
