Total shot in the dark here, but try unhiding the extensions for known file types and see what that file above your .mds is... I kind of wonder if winrar is associated with .mdf for some weird reason (and that seems to be the only one I'm seeing that doesn't actually end in .rar or .rar.001 or something in your pic).
Here's how to do so, if you don't know:
Un-Hide extensions for known file types under Windows XP and Vista
Hiding the file extension makes it impossible to identify the type of file you are working with. Hiding this information makes work more difficult. Showing the 3 character filename extension has no downside and no security risk. Hiding useful information makes no sense. You need to know the file type for certain 1099 Express operations. To un-hide:
Start Windows Explorer (There are several ways to start Windows Explorer.)
1. Use a normal right click the Windows Start Button (bottom left of screen) then click "MY COMPUTER" or "Computer" under Vista.
2. Left Click the Windows Start Button, then click "Explore".
If using Windows Vista the menu bar must be present. Click the "Organize" button, then click "Layout", then click "Menu Bar" if there is no check mark beside "Menu Bar".
Click on the VIEW MENU then click on Details.
Click on the TOOLS MENU, then FOLDER OPTIONS, then the VIEW TAB.
UNCHECK "Hide extensions for known file types"
Click "Apply to All Folders".
Then Click the OK Button.

I wasn't asking for that, I meant in your second pic. I thought I saw .rar at the end of "dot hack Infection part 1 [SLUS-whateveritwas]", and was wondering what the extension on the file above the .mds was/if winrar had somehow been associated with .mdf files. Of course it seems I was mistaken, so
Is that same thing in both archives you have pictured in that second pic, by chance, or just one?

You already had your .iso file back at this step, see the image I'm attaching. It's the file that's 2,598,567,936 bytes. Use that, and you're done.
You didn't notice it because you have Winrar associated to .iso's and you have Windows hiding known file extensions... Fixing the Winrar associations is a good idea. Open Winrar, select Options>Settings>Integration, and untick ISO

No. That's the game, you want that file and nothing else. Load it into PCSX2, or burn it to a DVD with ImgBurn if you're playing on a modded PS2.

Ok i get a wierd black screen when i try to start the game it says PS2 then black screen
You're getting into setting up PCSX2 at this point. And I'm close to no help with that (I can't run PCSX2) A quick google of 'pcsx2 dot hack infection' turns up threads on the official PCSX2 forums though. You might get some ideas if you skim over those.

Have you followed the setup guides on PCSX2's website?
Also, check the compatibility list, also on the website, to see if that particular game is working on the current build of PCSX2.
