Not so tough as insanely stupid/badly engineered, Shadow of the Beast. Or whatever it was named, along with its sequel.

I had a hard time playing Magna Carta: Tears of Blood. The battle system makes me want to kill myself.
I love Ecco the fucking Dolphin. But it is indeed pretty damn difficult.
The Genesis port of Hellfire is pure, unbridled evil. The arcade game isn't so bad, as you get to continue as soon as you die, but the Genesis version uses R-Type-esque restart points. And, on every level beyond the first, a single death is effectively game over, as there's certain sections that are simply more or less unbeatable without good weapons, and you're forced to play through them without making a single mistake, as a single hit returns you to the last restart point. I really like difficult shooters, but it's just silly, and not very fun as a result.
For a game that's challenging in just the right way, though, I'd say Alien Soldier. It's extremely tough, and rarely lets up, but it's incredibly fair and well balanced. Unlike so many other difficult games, it really feels like a test of your skill, rather than your ability to luck your way through moments of incredible cheapness, and it's extremely fun to play as a result. I died a lot while playing through it, but I don't think I can ever recall dying in it and feeling as if it wasn't my fault for having done so. I never became even remotely frustrated with it as a result. And dying only sent me back to revise my strategy and have another go. Wonderful, wonderful game.
