To rip mixed-mode games so you get the correct indexes in the cue files (if you prefer a cue/bin image) just rip the disc to CloneCD format using either CloneCD or Alcohol 120% and make sure that they are set to read the subchannel for both audio and data tracks (for Alcohol just choose the Play Station setting). Then mount the CloneCD image and rerip it using CDRwin. This works best since CDRwin has trouble reading the correct indexes when reading the disc straight from most drives. You could also do this with Alcohol images.

If it has audio tracks. Mixed-mode discs are discs that have both data and audio tracks in the same session.
