I'd have to say Valkyrie Profile for PSX (both story and gameplay wise)
Resident Evil 3: Nemesis. Because both Jill and Nemesis are great :p PSX version.
MGS 3 for PS2 (story wise)
Zone of Enders: The 2nd Runner for PS2 (gameplay wise)
EDIT: I absolutely loved Parasite Eve series. I demand a sequel damnit !

Sorry, that sure is more than 1 favourite game and I'm pretty sure there are more. These things are pretty difficult to limit as new games continue to hit the gamer market.

lots of games for each system I ever played,but if I have to choose just one game
for each system...I can't,so I'll name several games (in no particular order):
atari 2600: frogger and crackpot
nes: megaman 5,dejavu,smb and smb3
snes: FINAL FANTASY 6 (celeeeees!!!),mortal kombat 2,tales of phantasia
pc: DOOM,diablo 2,deus ex,zak mckraken & the alien mindbenders,
both crusader no remorse and crusader no regret,final fantasy 8 (ahhh,when I think
about the story behind eyes on me and about the ending and the scene when squall
meets rinoa and...ahhhhh,sniff sniff boooo hoooo sniff sniff),legacy of kain blood omen,(I HATEEE all the other episodes)...bla bla bla
psx: XENOGEARS (the second best rpg ever just behind ff6,at least in my list,I had
a crush on elly,she reminded me of celes,and I had a crush on celes too of course ) and Lunar (now I have a crush on luna,lol),ah,silent hill and...hmm,that's it.
of course all the megaman main series (megaman and megaman x,not that legends
thing or the non true megaman games),and all the 2d verticall scrolling shooters
like raiden. (I've spent DAYS AND DAYS of fun playing raiden 2).
well,I know the topic was about a game,but it's impossible to name just one if
you played thousands of games for a lot of systems,im sure it happens to everybody
else that they can't name just one.

I dont know why but I loved Rayman 2: The Great Escape. I still play it to this day

I recognize it as a great game, but the later levels were becoming too hard for my non-gamepad wielding platformer skillz. You should try Psychonauts, it reminds me of Rayman greatly, just... a lot more wacky.

My absolute favourite game... hard to decide. I'd say Fallout 2 for now.
