Picked up a couple of PS3 physical cheapies today as EB have another of their sales going:
Saints Row 2.
Assassin's Creed.

Here the case only held 3 discs, and the 4th disc was in a plastic wallet in front of the manual.

I have the Asian release Full Japanese/English options (even the manual is multi-language), large 4 disc case, NTSC-J but works on PAL region consoles (not NTSC-U though afaik).

Yeah that's what I read too. Boiling and cleaning it did the trick for me, though the drive still audibly struggled between 20 and 50%. Very strange. I've got a few games that came with discs in wallets but I've never had that problem before.
