Spider Lair is quite odd...it's a mini story that has nothing to do with any of the characters from the main game.

Try Pavel and Spider Lair dude.

The Pavel and Spider Lair ones are probably the best iirc. Sniper Team is alright. The Developer stuff is just a shooting range and waves of enemies. The Anna one fits right in with the main story....

It's not that bad really, pretty hard because of the amount of elite players around though. Not a huge amount of people playing it but sometimes 300-400 at night here, so I guess they're mostly NA...

Not really possible. Good chance a few uber elites would join the game.
You can join games with your friends but it's all public. So you'd need 20 something friends to fill a game, I assume that...
