Use Daemon Tools, (it will create a virtual CD-DVD drive in your comp) and mount the mds file with it. Now run the emu and play the game as if were in the virtual drive.
Alcohol 120%'s native image format. Mount it with that program, and run as disc. Or you could mount it an re-rip it into BIN/CUE so it'll work with ePSXe natively.

Mount the game with Daemon Tools and the run it in ePSXe from File > Run CDROM

Just out of curiosity, is there a way to directly convert a MDF into an ePSXe-readable format with Alcohol 120%? What I've been doing is mounting the MDF to a virtual drive and then making an CCD using that virtual drive as the source, but I wonder if there is a more direct way.
why not just mount it in the virtual drive and run it as a cd. anyway the way your doing it is quite direct and takes about the same amount of time as any other way.
I use isobuster to convert images to bin/cue. IMO bin/cue is the way to go, for emulation or straight up burning.
