Yes, you do. You'll also need to rejoin the split .i00/.i01 files to patch it. To do that simply, mount the .mds and re-rip a normal single part iso

You have to extract with 7zip, not winrar.
Once you've extracted the image file, you can ply it on a modded PS2 console or you can emulate it if you comp is powerful enough and the game is actually emulated:

Join the iso parts with HjSplit by renaming them like this: .i00 to .iso.001 and .i01 to .iso.002.
