What video plugins are you using, and how are you loading it? If it is an ISO what format is it in?

You cannot load MDS files in ePSXe.
What you need to do is emulate a cd-rom drive, and put that image file in the emulated drive. For that, you'll need a small software called Daemon Tools, which can be found here.

After you downloaded it and installed it, come back here for more help.
P.S. You'll obviously need admin rights to install that.

Are you sure you selected the correct CD drive in the CD plugin?

Use ePSXe CDR WNT/W2K core 1.5.2. and in the configuration make sure you select the correct drive letter that the emulated drive of Alcohol 120% is. If you have it set to FirstCDrom it probably won't work.
